DayDescriptionPOSITION PURPOSE:
The Housing Support and Benefits Specialist will serve as the Supplemental Security Income/Social Security Disability Insurance Outreach Access Recovery (SOAR) coordinator and Community Behavioral Health Rental (CBRA) subsidy specialist.
This position is responsible for increasing access to the disability income benefit programs administered by the Social Security Administration (SSA) for adults who are experiencing or at risk of homelessness and have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder.
The Housing Support & Benefits Specialist will also work to improve housing outcomes by increasing exits to or retention of permanent housing through the Community Behavioral Health Rental Assistance (CBRA) program, and in partnership with Housing and Recovery through Peer Support (HARPS), or other highly-vulnerable persons with complex behavioral health needs, to have opportunities to live independently in communities of their choice.
This position reports to the Hospital Diversion Program Manager.
MAJOR RESPONSIBIITIES:
SOAR :
Works with the referral sources and community partners to identify candidates through team meetings, outreaches, and referrals.
Initiates paperwork with consumers as referred to program by filing initial documentation of representation with SSA office.
Completes interviews with consumers to gather information to complete SSI/SSDI applications.
Gathers medical records and other information to complete SSI/SSDI applications.
Writes SOAR Medical Summary Reports for consumer applications.
Accompanies consumers to appointments at the Social Security Administration.
Coordinates visits to medical doctors, psychiatrists, and other specialists to obtain evidence for case.
Coordinates case management services with partners and help with providing case management services to consumers when needed.
Works with the homeless outreach and housing teams, providing consultations regarding SOAR process.
Participates in SOAR statewide strategic planning meetings.
Responsible for receiving referrals, screening for potential SOAR participants, completing SOAR applications, Online Application Tracking (OAT) and developing key contacts in the community to offer SOAR technical assistance.
Conducts outreach and engagement with individuals identified by homeless outreach teams, conducts outreach in partnership with existing housing programs, and links individuals to mental health services, housing services, and additional services as needed.
Assess individuals from the referral list to determine which services and resources are identified as needed using a person-centered approach.
Assist the team with administrative tasks as needed.
CBRA :
Providing service-planning support and participating in person-centered plan meetings at redetermination and/or revision plan meetings as needed.
Determines tenant initial and continued eligibility; calculate tenant rent for admission, interim/special and annual re-examinations with accuracy, efficiency and in compliance with DOC, ASO (whoever else), HUD other applicable Federal, State, Local, Fair Housing, Washington State Landlord/Tenant laws, procedures, policies and regulations, etc.
Prepares and execute tenant rent adjustments, repayment agreements, compliance & termination notices for occupancy, program lease violations.
Educates and inform applicants of program requirements and responsibilities.
Applies knowledge of residential lease contracts to educate clients of their rights and responsibilities.
Documents all client contacts, care and response, maintain records and updated rosters collect data and prepare reports as mandated by program procedure within agency standards and timeframes.
Utilizes Homeless Management Information Systems (HMIS) to record CBRA participants.
Maintains and updates HMIS database for CBRA referrals and participants.
Coordinates and linking the recipient to services including primary care and health homes; substance use treatment providers; mental health providers; medical, vision, nutritional and dental providers; vocational, education, employment and volunteer supports; hospitals and emergency rooms; probation and parole; crisis services; end of life planning; and other support groups and natural supports.
Provides entitlement assistance including obtaining documentation, navigating and monitoring the application process, and coordinating with the entitlement agency.
Assists with accessing supports to preserve the most independent living, such as individual and family counseling, support groups, and natural supports.
Provides supports to assist the individual in the development of independent living skills, such as skills coaching, financial counseling and anger management.
Provides supports to assist the individual in communicating with the landlord and/or property manager regarding the participant's disability (if authorized and appropriate), detailing accommodations needed, and addressing components of emergency procedures involving the landlord and/or property manager.
Coordinates with the tenant to review, update and modify his or her housing support and crisis plan on a regular basis to reflect current needs and address existing or recurring housing retention barriers.
Connects the individual to training and resources that will assist the individual in being a good tenant and remaining in lease compliance, including ongoing support with activities related to household management.
Attends scheduled interdisciplinary team meetings or clinical supervisory sessions to discuss quality of client care.
Maintains regular communication with tenants, landlords, community partners, funders, and management.
Conducts housing inspections.
Other duties as assigned.

QualificationsQUALIFICATONS:
Washington State Dept of Health (DOH) Agency Affiliated Counselor is required.
High School diploma or GED required.
Experience working with individuals with behavioral health needs.
Experience working with landlords and property management agencies.
Experience as a Certified Peer Counselor preferred.
